The gift voucher is issued in either paper or electronic form.

Each voucher has a unique code generated by POWAB Clinic following the Client’s order for a service provided by POWAB Clinic.

The issuance of a voucher is conditional upon full payment for the service offered by POWAB Clinic and acceptance of these terms and conditions.

An electronic voucher is generated after payment is made. The electronic voucher represents a specific monetary value. It may also be issued in paper form.

A paper voucher may represent either a specific monetary value or a specific treatment.

Gift vouchers are available for continuous sale and can be issued for any amount.

The validity period of a voucher is 12 months from the date of purchase. The expiration date of the voucher cannot be extended.

At the time of purchasing a service from POWAB Clinic, the Client must declare whether they wish to receive a voucher.

If the voucher is not redeemed within its validity period, the Client loses the right to use the purchased service covered by the voucher.

The Client is not entitled to exchange the voucher for a cash equivalent.

To use the voucher, the Client must book an appointment for the service covered by the voucher. A voucher can only be used once.

When booking an appointment using a voucher, please inform the staff that the voucher will be used as the form of payment for the service.

If the appointment is not cancelled at least 24 hours in advance, the voucher will be considered redeemed.

When redeeming the voucher, the Client must provide its unique number and always bring the voucher to the appointment.

The voucher can be used partially. No cash change will be given for a voucher exceeding the value of the service. If the service exceeds the voucher’s value, the Client must pay the difference.

Payment using a gift voucher cannot be combined with promotions or discounts.

Personalized (name-assigned) gift vouchers are non-transferable.

Vouchers for selected services may be exchanged for any other service offered by POWAB Clinic, except for radiofrequency treatments, tissue stimulators, and procedures performed by a doctor.

Value-based gift vouchers entitle the holder to choose any treatment, excluding radiofrequency treatments, tissue stimulators, and procedures performed by a doctor.

Gift vouchers are sold directly at POWAB Clinic and through the online store on the website powabclinic.pl. They can also be purchased by placing an order via the contact form available on POWAB Clinic’s Facebook and Instagram pages, through the Booksy platform, or by emailing [email protected].

Payment for a voucher purchased directly at POWAB Clinic is made by the Client in cash or by card.

For vouchers ordered online, payment is accepted via bank transfer, debit card, or credit card. The Seller confirms the order and provides bank transfer details via email.

For payments made via bank transfer, an appointment for the treatment can only be booked once the funds have been credited to our account.

The cost of shipping the gift voucher is borne by the Buyer.

Any complaints regarding improper fulfillment of obligations by POWAB Clinic as defined in these Terms and Conditions should be submitted by the Client directly at POWAB Clinic within 3 days of the service being provided.

The Client will receive information about the outcome of the complaint via email.

The Data Controller of the Client’s personal data is POWAB GROUP, ul. Grzybowska 97, 00-844 Warsaw.

Personal data is processed with appropriate security measures that meet legal requirements.

Personal data will be processed for the following purposes:

a) fulfillment of the voucher,

b) billing and invoicing of services,

c) handling of complaints (if applicable),

d) marketing purposes, if the legal basis for processing is the legitimate interests of the personal data controller – Article 6(1)(f) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR).

Additionally, personal data may be used for statistical purposes within the legally permitted and justified interests of the data controller (Article 6(1)(f) GDPR).

Providing personal data is voluntary but necessary for the fulfillment of the voucher.

Personal data will be stored only for the period necessary to fulfill the above purposes, and afterward for the duration required by legal obligations imposed on the data controller, including (i) provisions related to civil liability for proper performance of contractual obligations, and (ii) tax law provisions.

The Client has the right to: access their personal data, rectify it, delete it, restrict processing, transfer data, object to processing based on specific situations or for direct marketing purposes, and to withdraw consent for data processing at any time without affecting the lawfulness of processing based on consent before its withdrawal.

If the Client believes that data protection regulations have been violated, they have the right to lodge a complaint with a supervisory authority, particularly in the Member State of their habitual residence, place of work, or the place of the alleged violation.

The legal basis for processing personal data includes Article 6(1)(a), (b), (c), and (f) of the GDPR.

These Terms and Conditions may be amended at any time, provided that the previous provisions shall apply to vouchers purchased before the changes, unless the changes are more favorable to the Client.
